3-pack tickets must be issued to the same person (same name and address). A maximum of three names can appear on the tickets and each name will appear on all three tickets. The name(s) of all buyers must be on the original order form. The tickets issued will match the original order form.
If the 3-pack of tickets was ordered as a gift, the purchaser’s name and address will be listed on each ticket as the purchaser. Once the purchaser receives the tickets, they can decide who will receive the ticket as a gift. Remember: if one of the three tickets should win, it will have the purchaser’s name and not the name of the person to whom it was given as a gift. All three tickets will be issued in sequential order – no exceptions.
Having the purchaser’s name on all three tickets indicates who is legally responsible for the tickets and helps to ensure a clear audit trail of how many tickets have been sold.
It is up to the purchaser’s discretion to transfer any prizes won to the person(s) to whom they gave the ticket(s). The CHEO Foundation, Miratel Solutions Inc. and MNP LLP are not responsible for any disputes arising from a failure or refusal to transfer winnings and prizes, nor will they mediate or negotiate any settlement should a conflict arise.
